Case Manager Job Summary:
  • The Case Manager plays an integral part in the coordination of care assessing the member’s condition and providing them the necessary resources, options, and coordination for a healthy care outcome.
  • The RN Care Manager is primarily responsible for identifying members appropriate for targeted, high touch community-based integrated care coordination services.
  • The Case Manager compiles a comprehensive care management assessment and gathers pertinent information about the members needs by interviewing the member, appropriate family members, physicians, ancillary health professionals and others as necessary.
  • They will participate in interdisciplinary conferences to review clinical assessments, update care plans and determine follow-up frequency.
  • Ongoing coaching, monitoring and evaluation of care plans will be done face to face with the member, telephonically or electronically through email, among members and Providers.
  • The Case Manager also acts as an advocate for members and their families linking them to other members of the care team to help them gain knowledge of their disease process and to identify community resources for continued growth toward a maximum level of independence. 
Case Manager Background: 
  • Valid, unrestricted state Registered Nurse (R.N.) license.
  • Bachelor's Degree in Nursing or healthcare-related field is a plus.
  • Certification in Case Management (CCM) is preferred.
  • Around 3-4 years’ acute care clinical nursing experience, including:
  • At least 2 years’ experience with Care Coordination, Case Management, Utilization Review and/or discharge planning.
  • Case Management experience will ideally be from a Health Plan or TPA.
  • Experience working with Federal and State assistance programs (Medicare/Medicaid/etc) is preferred.
  • Basic computer literacy (MS Office) and typing skills are necessary.
  • Bilingual in English and Spanish (or other languages beneficial to local population) is a plus.
