Social Worker Job Summary:
  • Work in collaboration with an interdisciplinary team to identify and work toward timely resolution of issues related to psycho-social needs, including assistance with financial issues, transportation and community support services.
  • Assist in the coordination of the member’s identified psycho-social needs, utilizing community resources and support when appropriate.
  • Collaborate with external utilization management personnel and the community in identifying members who could benefit from case management services and to actualize service plans.
  • Provide social work consultation and education to members and their families and act as an advocate when needed.
  • Develop and maintain a network of current community resources and services where members can be referred for assistance.
  • Analyze from assessment in order to identify individual issues, problems, and resources that are most appropriate to meet the member’s needs.
  • Evaluate and anticipate member’s needs, provide support, maintain a role as the member’s liaison while forecasting independence and decision making on his/her part. Based on the member’s assessed needs, make appropriate referrals.
  • Collaborate and communicate with the member, family, significant other(s), physicians, and other health care providers to accomplish goals identified on the service/care plan.
  • Participate in pro-active identification of patients appropriate for care management.
  • Initiate appropriate follow-up care and develop individual care management service plans incorporating assessment, education, resource planning and coordination of services of those patients accepted for case management.

Social Worker Background: 
  • Requires Master degree in Social Work (M.S.W.); with additional years of supervised/documented experience.
  • At least 2 years’ experience working with the elderly and completing home visits.
  • At least 1 years of experience with Care Coordination/Case Management and Discharge Planning.
  • Intermediate computer literacy (MS Office) and typing skills are necessary.
  • Bilingual in Spanish, or another other language used by local populations, is always a plus.
