ER Care Coordinator Job Summary:
  • The RN Care Coordinator is primarily responsible for monitoring and reviewing electronic ER alerts and identifying patients that may benefit from a care coordinated discharge directly from the hospital Emergency Department (ED), if appropriate and safe. 
  • This position will also be responsible for supporting the Clinical Services Department and include concurrent review activities as well as assisting the Care Management team. 
  • Performs an initial ED review utilizing the reference tool to identify diagnosis that have been identified as having a higher probability of rendering potential coordination of discharge from the ED in order to avoid unnecessary hospital admissions.
  • Responsible for assessing and identifying members who are appropriate for palliative care or hospice conversion.
  • Responsible for clear, concise, concrete objective documentation of relevant communication with ED provider, staff and/or hospitalists of options to facilitate discharge from the ED.
  • Prepares daily, weekly, monthly, and quarterly reporting on eVIGILS alerts received and outcomes associated with all ED care coordination efforts.
  • Back-up for reviewing concurrent hospital admissions, status changes and aberrant reviews. 
  • Participates in weekend, on-call coverage for Clinical Services Department.
ER Care Coordinator Background: 
  • Valid unrestricted state Registered Nurses (R.N.) license.
  • Prefer candidates that have a Bachelor of Science in Nursing (B.S.N) or related field.
  • Roughly 3-5 years of hospital, acute care clinical nursing experience.
  • At least 1-2 years’ experience with Hospital Emergency Room Case Management or Utilization Review.
  • Knowledge of Medicare, State and local managed care regulations, prior experience in Managed Care setting preferred.
  • Strong oral and written communication skills; ability to interact within all levels of the organization as well as with external contacts
  • Possess planning, organizing, conflict resolution, negotiating and interpersonal skill.
  • Familiarity with electronic health records (EHR) systems, preferably eVIGILS.
  • Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously.
