** Primarily In-Office/Telephonic Outreach (75%), with some travel (25%)
to established healthcare facilities for In-Person Assessments. **
Social Worker Job Summary:
  • The Social Work Case Manager will "touch-base" with high-risk members of the health plan.
  • Responsible for performing telephonic and field case management within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.
  • Uses appropriate screening criteria, knowledge and clinical judgment to assess member needs.
  • Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
  • Monitors and evaluates effectiveness of care plan and modifies plan as needed.
  • Supports member access to appropriate quality and cost effective care.
  • Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.
Social Worker Background: 
  • Requires Master degree in Social Work (M.S.W.); additional years of supervised/documented experience is a plus.
  • Prefer a valid, unrestricted state license as a Licensed Clinical Social Worker (L.C.S.W) 
  • Requires 2 years of clinical social work experience
  • At least 1 years of experience with Care Coordination/Case Management and Discharge Planning.
  • Intermediate computer literacy (MS Office) and typing skills are necessary.
  • Bilingual (English/Spanish or English/Hmong) is always a plus.
