Director of Case Management Job Summary:
  • Develops and directs case management programs that support patients and populations, focusing on safety, quality, experience, access and appropriate utilization of resources for their optimal health outcomes.  
  • Collaborates with operations and clinical leadership teams to establish strategic initiatives and key performance indicators. 
  • Accountable for developing and maintaining an ongoing measurement and evaluation plan for all Integrated Care Management goals and developing programs that ensure high-quality clinical outcomes for patients of care management programs.
  • Responsible for the achievement of total cost of care targets for patients and populations receiving care management services and high patient or member satisfaction with care management programs. Provides care management best practice recommendations to the Integrated Delivery Network.
  • Works collaboratively with nursing, clinical programs, clinical services, compliance, risk management, providers, operations, health plans and the community to understand trends, to improve care and foster an interdisciplinary approach to Integrated Care Management across Intermountain.
  • Stays abreast of changes to the healthcare environment, care management and healthcare reimbursement innovations through local, regional and national networking.
  • Supports best practice through policy standardization, appropriate data collection and analysis, evaluation of EMR tools and applications, information system review, analysis and recommendations for optimal support and clinical education collaboration.
  • Leads and supervises case management professionals and is responsible for staffing, performance management and development of those team.
Director of Case Management Background
  • Valid, unrestricted state Registered Nurse (R.N.) license and Master's degree in Nursing or related field.
  • At least 4-5 years clinical nursing experience at an acute care hospital.
  • Around 3-5 years Case Management experience; Certified Case Manager (CCM) preferred.
  • At least 4-5 years managerial experience and leadership of a Case Management staff within a hospital environment.  
  • Ability to analyze and integrate information and make sound decisions based upon established guidelines.
  • Resourcefulness as demonstrated by the effective application of professional knowledge to new situations. Possess planning, organizing, conflict resolution, negotiating and interpersonal skills.
  • Experience in training / teaching staffs to meet operational requirements and goals.
  • Strong oral and written communication skills; ability to interact with all levels of internal and external contacts.
  • Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously.
